to the Project and that he acted contrary to the Bylaws of Engineers and Geoscientists BC. Mr. Van Dyk agreed that he: ◻ relied on incorrect drawings of the formwork and falsework in carrying out his inspection when he knew or ought to have known that the drawings were incorrect; ◻ failed to inspect the formwork and falsework immediately before the concrete pour as required by the Occupational Health and Safety Regulation; ◻ authenticated a field review report on January 4, 2021, when he had not inspected the formwork and falsework since December 23, 2020; and ◻ failed to create or maintain adequate documentation of his inspection of the Project site to prepare the field review for the Project. As outlined in the Consent Order, Mr. Van Dyk must successfully complete educational courses and provide a declaration that he has read and is familiar with two Engineers and Geoscientists BC guidelines. Six months after the suspension period concludes, Mr. Van Dyk must undergo a practice review conducted by the Engineers and Geoscientists

In a June 27, 2024, Consent Order, John Van Dyk, P.Eng., admitted that he demonstrated unprofessional conduct in relation to the engineering services he provided for the inspection and approval of formwork and falsework for a building in Colwood, BC. As part of the Consent Order, Mr. Van Dyk's registration with Engineers and Geoscientists BC is suspended for a period of three months. The suspension period contracted as a field engineer to conduct a prior-to-pour inspection of the formwork and falsework for part of a building in Colwood, BC (the Project). During a December 23, 2020, inspection, Mr. Van Dyk noticed differences between the Project drawings that were provided to him and the Project site but completed his inspection despite the inconsistencies. On January 4, 2021, Mr. Van Dyk authenticated a field review report based off his December 23, 2020, inspection without doing a second inspection. The formwork and falsework collapsed during the concrete pour. In the Consent Order, Mr. Van Dyk admitted that he demonstrated unprofessional conduct related started on June 28, 2024.
