It’s important to consider the sociotechnical factors by recognizing the interaction between people and technology in workplaces. In other words, consider how people may behave around equipment and how that might affect its use, he said. “When you see something in a state that it’s not supposed to be in, ask how did

it get that way?” he said. “For us, that’s some of the most challenging evidence to collect, to make sense of. It’s also in many cases, just like in this, one of the most important contributing factors.” Another key learning for all engineers is not to counsel unqualified workers. Only people who have the necessary skills and knowledge should be participating in hazardous work, including end-of-life dismantling and decommissioning. “If a job involves hazards and you’re not committed to doing the job yourself, the response is to get somebody in there who knows it and will do it properly,” he said.
